Waist-to-height ratio

Waist-to-height ratio (WHtR) measures abdominal fat distribution: keep your waist under half your height. WHtR ≤ 0.5 is healthy; 0.5-0.6 is concerning; over 0.6 is high-risk. Better predictor of metabolic disease than BMI in nearly every study.

Definition

Waist-to-height ratio (WHtR) measures abdominal fat distribution: keep your waist under half your height. WHtR ≤ 0.5 is healthy; 0.5-0.6 is concerning; over 0.6 is high-risk. Better predictor of metabolic disease than BMI in nearly every study.

What it means

Why WHtR matters: visceral fat (around organs, in the abdomen) is metabolically harmful. Subcutaneous fat (under skin, in hips/thighs) is less so. BMI doesn’t distinguish; waist circumference does. The 0.5 threshold means “keep your waist under half your height” — easy to remember, applies across sexes and ethnicities, and predicts heart disease, type 2 diabetes, and overall mortality more reliably than BMI alone. Multiple meta-analyses (Ashwell et al. 2012, Browning et al. 2010) show WHtR predictive of metabolic syndrome 1.5-2x more accurately than BMI.

Why it matters

If you’ve ever wondered why a thin friend has high cholesterol or a muscular friend doesn’t look fit on paper but has perfect metabolic health, WHtR distinguishes. Practical use: at the start of a fitness or weight-loss program, measure waist circumference. Over weeks of training, BMI may not budge much (muscle adds weight while fat is lost), but WHtR drops as visceral fat decreases. WHtR is also more responsive to recent training: even a 3-month exercise program can drop WHtR before BMI moves at all.

Example

70-inch tall (5′10″) person with 35-inch waist: WHtR = 35/70 = 0.5 (borderline healthy/risk). Same height with 38-inch waist: 0.54 (concerning). Same height with 32-inch waist: 0.46 (healthy). The 0.5 threshold is universal across sex and ethnicity.

How do I measure waist correctly?

Stand relaxed, exhale normally. Wrap measuring tape around the smallest point between the lowest rib and hip bone (typically just above the navel). Don’t pull tight or suck in. Measure twice for accuracy.

What if my WHtR is high but I look fit?

‘Skinny fat’ — low overall body fat but disproportionately concentrated in abdomen. Common in sedentary people who diet without exercising. Add resistance training to build muscle and reduce visceral fat specifically.

Does this work for kids?

Yes for school-age children and teens. Adult WHtR threshold (0.5) applies to children age 6+; younger children have different patterns. Check pediatric guidelines for under-6.
